Polk Community Free Clinic At Trinity Lutheran Church – The low income and uninsured may receive basic care. The church has an Acute Care Clinic offering acute and chronic care for illness. This can me medical help for diabetes, cancer, or other conditions. The primary care clinic is currently taking new patients, but patients must go through an intake appointment to set up an appointment with a doctor. Please be advised that the time to get medical care may be several weeks due to demand and the application process.
