They support seniors that live in Chariton, Lafayette, Saline, Johnson, Pettis, Carroll, Henry, Benton, Bates, St. Clair, Hickory, Cedar, or Vernon Counties. Resources are available as clients continue to live independently and cope with all of the accompanying challenge they face. Programs are run at the various local Senior Centers in those counties.
Applicants will go through an Individualized Evaluation and Assessment. Based on the results, there will be referrals to programs such as financial assistance, medical care, transportation, meals, housekeeping, personal care, telephone reassurance, and friendly visits done by volunteers. Often time there is information provided about services seniors may be unaware of. Of particular focus is education on how people can remain in their own home in Carroll or Vernon County and also make long-term plans.

The Family Caregiver Support Program recognizes that no one plays a more important role in helping older person remain independent than a caregiver. The Care Connection for Aging Services six-week program is conducted by people who also live with chronic diseases and conditions.
Topics of interest from clients in Missouri include the management of arthritis, diabetes, depression, asthma, bronchitis, heart disease and emphysema. Workshops also help people address ways to deal with symptoms. This includes healthy nutrition and exercise; proper use of medication; and working with care providers. The Regional Long-Term Care Ombudsman Program’s volunteers are trained in working with nursing home residents and advocating for their rights. The program serves those who reside in nearly 90 nursing homes in counties such as St. Clair, Carroll, Pettis and others. Otherwise known as the Care Connection service area.
Under the direction of staff from Care Connection, the statewide Senior Medicare Patrol Service empowers and assists Medicare beneficiaries, their families, and their caregivers. It works by preventing detecting, and reporting health-care fraud or billing errors. This is accomplished through the provision of counseling, outreach and educational services.
Care Connection’s licensed Health Insurance Marketplace navigators provide free, unbiased information about private insurance plans as well as various Affordable Care Act policies. They also do much more than that. They will assist people of all ages with the Missouri Health Insurance Marketplace enrollment process; calculate tax credits, and also determine which plan best fits each individual’s specific needs and financial situation.
Care Connection partners with Community Leaders Assisting the Insured of Missouri (CLAIM), which is the official State Health Insurance Assistance Program. Together they work to provide reliable information and assistance related to Medicare and Medicaid.
There are dozens of certified volunteers, care managers, and insurance counselors in centers in the region. They are all trained in providing assistance Eligibility, Enrollment, Tips on how to access Medical and Prescription Benefits, Medicare Advantage (Part C) and supplemental Medigap Plans, and options such as Medicare’s Parts A (hospitalization), B (outpatient and physicians’ care) and D (prescription drug) Plans
In-Home Services provide the following services for seniors who are no longer able to perform basic household tasks. Staff from Care Connection for Aging Services can help Benton, Lafayette and other families with Housekeeping, Medical Alert System Set-Up, Personal Care, and also Telephone Reassurance.
Care Connection contracts with the OATS bus service and other local companies to provide rides to the senior centers in Missouri. This will be for services such as meals and activities. There are also rides to to grocery stores, department stores, and pharmacies. Other transportation for the elderly may be to medical appointments at clinics, doctors’ offices, and hospitals. Or residents can look into Provide-A-Ride, which enlists volunteer drivers to provide transportation to medical appointments.
