Polk County Florida Social Services programs.

The main programs provided by Polk County Social Services address medical needs. The county works to ensure that all families, whether they have insurance or not, have access to health care. There are also resources such as the Elderly Services Program, veteran resources, and applications to grants for paying energy bills, among other programs.

Support for senior citizens and veterans in Polk County

There are two main programs run, as noted below. They include Elderly Services, which is for disabled adults (18 to 59 years of age) as well as the elderly (60 years of age and older). Polk County's professional case managers coordinate and arrange social services to best address each client's physical, medical, environmental, social, emotional and mental needs. There are many components offered to this, as noted below.

Each form of assistance has the goal of preserving the individual's confidence as well as dignity and allowing them to remain in the least restrictive environment possible. Specific types of assistance are as follows. There is financial help for home energy bill assistance, Transportation, Adult day care services, Long-term and rehabilitative care, Respite care for caregivers, Emergency Alert response systems, Homemaking, and food/Nutrition.

Referrals are also given to seniors. They can learn about federal grants for paying rent or utility bills. There are also other resources such as applications to the Affordable Health Care Act, Medicaid, and more. Much of this is provided by the Polk County Social Services Elder Help Line as well as Information and referral service.

The Rohr Home is a licensed skilled nursing programs that is always highly rated by the American Health Care Association. It is located in a beautiful setting in Polk County Florida among the moss-laden oak trees as well as azaleas of Bartow Florida. Rohr Home is specializing in long-term and sub-acute care, however the facility also offers opportunities for residents to interact with staff and each other. This will allow the client to participate in daily activities such as birthday parties or special events.

 

 

 

This program is coordinated by a full-time staff, including nurses, activities director and social worker. Other services at the home include Dentist, 24-hour nursing care, Clinical lab, Pharmacy and dietician services, a Podiatrist, and Mental health care facility.

Guests of the home can access some medical care too. Some of this assistance in Polk County Florida includes Resident and family counseling, Hospice, Prescription Medications, Speech therapy, and basic needs from a Laundry on site and Beautician.

Polk County veterans are also a focus of the social service programs. For those residents that have served their country, or their family members, Polk County's Veteran Services is there for them. The county will help former, current and future military service members, regardless of their income or financial challenge being faced.

The assistance from a case manager will help the person wade through the often intricate and complicated process of obtaining government military benefits. Many of Polk County's Florida veterans are eligible to benefit from a number of federal and state of Florida public aid. Some of what is available is as follows.

-Life insurance for their family, including Survivor benefits.
-Compensation including applications to grants for bills, rent, and other veteran expenses.
-Education and job training as well as Vocational rehabilitation.
-Applications to Home Loans. This may be VA loan guarantees are made to veterans, reservists, service members and unmarried surviving spouses.
-Social Services will help people obtain a military pension.
-Free medical care and prescription drugs.
-Burial benefits from the state of Florida. Honorably discharged veterans and active-duty service members who have passed away may be eligible for a government-furnished headstone or marker; burial in a national cemetery; can be given a federal government presidential memorial certificate; or reimbursement for costs.

Polk County Social Services provides more general support too. Healthy Families helps build and assists residents. This was created to be a voluntary home visitation program that promotes the development of families, single moms, and children.

 

 

 

Healthy Families encourages positive parenting by all residents. It endeavors to reduce or eliminate negative outcomes, such as poverty, abuse and neglect. Outreach services from a case manager help create and support good parenting, trusting relationships, mother-and-infant bonding and attachment, healthy physical, job training, and educational development for children, and healthy parent-and-child interaction. The Polk County Florida program also referrals to be used for accessing available community services and resources for participating families.
