My name is Earl; I've been recently diagnosed with urethral cancer and I'm applying for temporary assistance because I'm unable to perform my duties at work at this time. I'm a Roofer and I've been having a lot of dizziness, and it's become too dangerous for me to remain on the job at the moment and way behind on all my bills.
I'm in the process of getting treatment so I can get back to work, I have a 6-year-old son who lives with me and I'm not really sure what to do. I've never needed to ask for help like this and don't know exactly how to get any help. This is very embarrassing and I'm terribly sorry for asking. I live in Hampton Virginia, 23669.
