Looking for help in Cleveland Ohio

Hi-I'm looking for advice on 2 fronts- I'm a 61-year-old single male living in Cleveland Ohio (44130-zip code). 2 years ago, I suffered massive heart attack and had operation and am now unable to work and living on 1320 disability benefits-just enough to cover bills -but I lack funds to eat properly. I receive snap-15 dollars a month-doesn't go too far. I carry 7500 balances on credit card debt and would like to get this amount in debt settlement to free up money for food- who's good to get debt settled or consolidated? secondly, what food programs are available to me in 44130 zip code to help me to eat properly.
