Lake County Ohio resources to pay bills needed

Hello,
My name is Jessica and I work for a home health agency covering Lake County and one of my clients who lives in Madison Ohio that is an elderly woman who has extensive medical issues and who is a veteran. She is in a very bad situation. All of her utilities have shut off notices and are about to be shut off and has an eviction notice and is about to be kicked out of her home. What makes this situation even scarier for her is the fact that she has zero living family and no children. I am desperately trying to find her help or find out what to do. PLEASE PLEASE...HELP!!...I anyway even if it is only to point me in the right direction of where I can find her help at. I cannot sit back and watch this woman go homeless and end up on the street.
