Will government give Samsung phone for free to replace broken one

Him I live in Ponchatoula, Louisiana 70454 in Tangipahoa parish. I’m a single mother of two children and am currently experiencing a hardship. My phone was shut off and I need it for work. I’m in need of assistance ASAP for a new phone or phone plans. Any information you can give me is appreciated for a Samsung or Android phone. My current application for a new phone seems held up and those wait list are currently closed. We receive food stamps and my children are on Medicaid which got us the Samsung Galaxy for free 2 years ago. Any other information needed please let me know.

To Whom it May Concern:
I have a Samsung Galaxy S16 phone that was gifted to me several years ago from the government and now it broke. The screen has been cracked over the years as the phone is old, and the screen now is not usable.
I am 71 years old, disabled, a SNAP and SSI/SSA recipient, who lives below the US poverty guidelines ($850 monthly).
I need greatly discounted, free unlimited 5G mobile phone service. As well as a new phone and Samsung would be great. Whatever G model is give out these days for free would be great as long as it is newer than my other one and it works. I also need a call plan for my current phone in Tulsa, Oklahoma.
Please advise me as to how to access an application for such services.
