Please accept the following written statement concerning my disability. For the past year and a half I have been in “constant” pain that radiates from my back through my hip down to my leg. It is a consistent sharp throbbing pain accompanied by numbness, burning, loss of balance, loss of bladder control and weakness in my left leg that has become increasing worse to the point I cannot perform routine daily functions nor work. I also have severe muscle spasms and pain that prevent normal sleep. I am also suffering from depression and anxiety attacks due to this condition. When the pain reached a level of me becoming being bed ridden I sought treatment at CMC Steele Creek emergency room on 04/28/18 where it was confirmed that I have Lumbar Radiculopathy, I was given two steroid injections, prescribed Cyclobenzaprine, Naproxen, and Prednisone and a referral to OrthoCarolina to a Spine Specialist. The following day I reached out to my area Department of Social Services to apply for Adult Medicaid, Food Stamps and to get information on applying for disability. After my appointment, I applied for disability online which is currently still pending.
On 06/02/18 I saw a doctor at OrthoCarolina for an initial visit then again on 06/05/18 who did an X-Ray that revealed that several of my bones from my back to my hip are directly on top of each other which is unusual as I was not involved in an accident or suffered any type of trauma. She recommended an MRI and prescribed Tramadol, Medrol Dose Pak, and Duexis. Her initial determination was that a combination of injections and surgery may alleviate some of the symptoms. She did conclude that this is going to be a long term process based on her initial findings in the X-Ray. I went for a follow-up on 06/12/18 to advise that the pain is becoming worse at which time she referred me to Pain Management for some relief however, at that time I had no means to pay for this option nor the medications prescribed.
On 06/18/18 I received a call from the Medicaid Office in Raleigh, NC (1-800-634-2986 x 6537) who basically advised me that I had no chance of being approved for Medicaid due to my age, work history and condition also, she discouraged me from going to the Emergency Room (hospital and mental health) stating they would not be able to assist me and I would just accumulate debt while attempting to validate my claim. She did however, suggest I look into alternative assistance like the sliding scale program even though the waiting list for most of the clinics is a couple of months and concluded the conversation by asking if I would object to seeing one of their doctors which I agreed to do but I never received a follow up call nor correspondence. I applied for Med Assist and the Care Ring Physician Reach out Program and was accepted in both.
I had my MRI on 07/30/18 and was then referred to Dr. for epidural steroid shots on 08/13/18 which did not alleviate the pain at all.
On 08/18/18 I went to an appointment for a psychiatric assessment with Dr. Warren Steinmuller scheduled by Disability Determination Services.
On 08/25/18 I received a Notice of Denial for Medical Assistance Retro and Ongoing in regards to Medicaid. I have filed for an appeal hearing and am awaiting a notice of the time/date.
My next appointment was a consultation with spine surgeon on 08/31/18 who recommended surgery to alleviate the current symptoms to be followed by long-term physical therapy. Up to this point I had submitted all of my treatment notes, doctor’s appointments, and bills to my Department of Social Services case worker Brandi Cochran and kept her updated every step of the way as advised when I applied. The last interaction was a phone call from her advising that she has nothing to do with my Medicaid determination and to stop sending her the verification however, I was not directed to send them to doctor in Raleigh nor anyone else. My surgery was scheduled for 09/24/18 at Charlotte Orthopedic Hospital and as the root cause was not addressed I received a referral to Dynamic Health and Pain Management (5215-A South Boulevard Charlotte, NC 28217)) where I received an independent x-ray from Dr. which revealed that I have congenital shortened pedicles causing stenosis along with damaged L4 & L5 disks. As this is a condition that was present from birth surgery alone will not be enough to resolve this issue. I will have to contend with this the rest of my life and take measures to get my body to a state of health that can cope with the pain that will always be a part of me. The recommended treatment plan costs $4,000 which I do not have readily available and is not covered by Physicians Reach Out. I have to take steps to treat the intense level of pain and train my body to get back to a state that I was prior to a year and a half ago when my body could make the appropriate adjustments. I had a follow-up appointment on 09/17/18 and at this time the surgery has been delayed until the problem at the heart of this can be addressed. This condition is ongoing and will require long-term treatment.
As of today’s date I cannot:

· Stand/Walk/Bend /Sit longer than 10 minutes
· Type/Write/Read for longer than 10 minutes
· Get dressed or bathe without assistance
· Prepare my own meals
· Do any chores
· Drive
· Handle my own business affairs
· Have a normal social or sexual relationship
· Participate in social activities
· Interact with my family and/or friends
· Engage in any hobbies or interests
· Concentrate on any one thing for any extended amount of time or remember parts of a day
I have no source of income.

I'm on SSI. I'm currently homeless with no income. I have been staying in hotels and motels past months. It’s too expensive to live in motels. Please help me get home. I receive 889 month and did not qualify for Medicaid. It’s not enough for security deposit and 1st month’s rent. Any suggestions on what TO DO? I've looked everywhere even SSI office, housing authority office, and Medicaid office. I don't know what else to do. The 1st is coming with no income. I will have about 1200. Still isn't enough.

That is a variety of resources you need. Try financial help from the government, and that link. We have by by state of health care, government programs like Medicaid, housing, and much more. SSI is also listed on the site.

Hello, I was checking out you site ad you didn’t' have anything for hearing loss for people who can't even get Medicaid. I've been denied 7 times for assistance with hearing aids and ear surgery and all I have left is my right ear 25% due to my cleft lip and palette. Please if you can share this on your site. I need all the help I can get. I can't lose my hearing. No job will hire me so I will have no income. I've been a nurse since I was 18. And now I can't even do what I love. Please look at my go fund me and see if you can help me.
