Co-pays for medical needs, including depression

I have severe pain from several herniated discs, compressed discs, neuropathy, spondylitis, stenosis and arthritic pain in my neck, thoracic and lumbar spine. At this point I am barely able to walk, however I am unable to afford to see my pain management doctor, or a rheumatologist, or afford my medication to help alleviate the pain.
My psychiatrist is worried as I am severely depressed and am unable to afford "Pristiq", (a $95.00 co-pay.) The "only" antidepressant of the 20 I have tried that works.
I have lost nearly all of my bottom teeth and because of the holes left where food gets caught, I have constant infections.
I am supposed to be on two heart medications, but cannot afford to either see the doctor, who is concerned with the potential of a stroke, or one medication that has a $95.00 co-pay.
I have attempted to get assistance through Needy Meds, and several other potential assistance programs, however none of the medications I need are covered by "any" of the programs that I have been referred to.
