Annual cost to raise a kid

Please do not include college costs, even though that would be needed (or some type of vocational/technical skill, etc.). Average cost from 0 to 18, knowing that some years more than others, and please factor in medical (such as birth, immunizations, etc.). So include everything...cost of more housing/space for a kid, furniture, medications, diapers (cloth or disposable), formula, clothes, toys, extra insurance, hygiene supplies, everything else.
Do not include cost of missed work or daycare, but obviously a single parent needs one or other and two parent one needs to either not work (or work differently) or they pay for daycare.
