Need to file for SSI

I'm 53 and has had multiple Strokes in the past 3-4 years. I have been under a Dr. care until my medical "Gold Coast" expired and I need help in filling out forms for SSI because I was denied due to missing info and don't have a way to get to ACE program in Oxnard. I have memory and thought problem due to stroke. I don't know what to do and I can't work so I'm basically homeless. I stay here and there but unsure of time.
