Help for heart attach patient

Had heart attack in December 2013 and now have no income. I applied for social security disability and all my utilities are behind or disconnected. My house phone disconnected, electric is two month behind, own my home but taxes unpaid, and no insurance on home. I had to park auto, my medicine prescriptions are not filled. Worked all my life and losing everything. Please help!
